The difficulty in prognostication of ILD patients has been quoted as a barrier to timely initiation of advance care planning in patients. This single centre study of 140 patients with ILD showed that the Surprise Question- ‘Would you be surprised if this patient were to die within 1 year?’ is a useful tool to guide initiation of advance care planning. More patients with ILD may be referred for timely palliative care with adoption of this simple tool.

This small mixed-methods pilot study of palliative co-management of patients with moderately severe ILD , using telehealth when needed, shows the possible ways of expanding access to advance care planning and timely opioids for symptom relief. There are useful insights in the qualitative arm of the study in the thematic analysis of interviews with patients and caregivers as well as clinician focus groups that can inform adaptation of similar models to different practice contexts and geographies.
