Dupilumab, Atopic Dermatitis, and Mycosis Fungoides

In this issue of JAMA Dermatology, Boesjes et al1 share their experience with patients with presumed atopic dermatitis (AD) who, shortly after dupilumab initiation, experienced worsening skin symptoms unlike their previous disease. Skin biopsies were characterized by atypical T-cell lymphocytic infiltrates. Although some of these patients developed mycosis fungoides (MF), most of their cases resolved spontaneously on discontinuation of dupilumab and were diagnosed with lymphoid reactions (LRs), a transitory drug-induced lymphomatoid reaction. This novel observation merits reflection, in view of several recent reports of cutaneous T-cell lymphoma emerging shortly after initiation of dupilumab.
