Cases of Mammary Paget’s disease without an underlying carcinoma are few and far between. These cases require extensive clinical exploring to reach a definitive diagnosis. A 74 year old woman presented with history of recurring eczematous lesion of nipple which was ultimately diagnosed as Paget’s disease. However extensive examining and investigations did not reveal any underlying tumor. First described in 1874 Mammary Paget’s disease is in itself an unusual presentation of breast cancer. Exceptionally, however, it may occur without any evidence of an underlying tumor. These cases usually have a better prognosis. Accurate clinical diagnosis requires thorough clinical examination and investigations. In the present case, repeated ulceration and bleeding pushed the patient to seek medical care. Clinical, pathological and radiological services were utilized to reach a definitive diagnosis and devise a treatment plan.
