Revision anatomic total shoulder arthroplasty can be a demanding procedure. We present a surgical technique of a liner exchange in a metal-back anatomic total shoulder arthroplasty performed through a subscapularis split approach in a patient with isolated eccentric liner wear. The described technique provided sufficient glenoid exposure for the liner exchange without the need for lesser tuberosity osteotomy, subscapularis peel, or subscapularis tenotomy, which enable early functional rehabilitation.
